Pregnancy Weight Gain: What's Normal and What's Not
Trimester-by-trimester guide based on IOM 2009 recommendations and pre-pregnancy BMI.
The IOM 2009 Guidelines
The current clinical standard for pregnancy weight gain in the United States is the 2009 report Weight Gain During Pregnancy: Reexamining the Guidelines, published by the Institute of Medicine (now the National Academy of Medicine). The report revised earlier 1990 guidelines based on a comprehensive review of evidence linking gestational weight gain to maternal and infant outcomes.
The IOM 2009 guidelines stratify recommended total weight gain by pre-pregnancy BMI category, computed using the standard CDC formula (BMI = 703 × weight[lb] / height[in]²) or equivalently kg/m²:
- Underweight (BMI < 18.5): 28-40 lb (12.5-18.0 kg)
- Healthy weight (BMI 18.5-24.9): 25-35 lb (11.5-16.0 kg)
- Overweight (BMI 25.0-29.9): 15-25 lb (7.0-11.5 kg)
- Obese (BMI ≥ 30.0): 11-20 lb (5.0-9.0 kg)
These ranges are total weight gain across the entire pregnancy, measured from pre-pregnancy weight to the last weight before delivery. They apply to singleton pregnancies. For twin pregnancies, the IOM recommends higher gains: 37-54 lb for healthy-weight mothers, 31-50 lb for overweight mothers, and 25-42 lb for obese mothers. The IOM did not issue twin guidelines for underweight mothers due to insufficient evidence.
Where the Weight Goes
A typical healthy-weight pregnancy produces a 30-lb weight gain distributed approximately as follows:
- Fetus: 7.5 lb
- Placenta: 1.5 lb
- Amniotic fluid: 2 lb
- Uterus (enlarged): 2 lb
- Breast tissue (enlarged): 1-3 lb
- Blood volume (expanded): 3-4 lb
- Extracellular fluid: 4 lb
- Maternal fat stores: 7-10 lb
Only about 7-10 lb of the total gain is maternal fat — and that fat is not "excess." It is the energy reserve the body builds to support the metabolic demands of late pregnancy and the roughly 500 kcal/day additional energy cost of exclusive breastfeeding in the postpartum period.
Trimester-by-Trimester Breakdown
First Trimester (Weeks 0-13)
The IOM 2009 guidelines recommend 2-4 lb of total weight gain in the first trimester for healthy-weight women, regardless of BMI category. This is because the fetus, placenta, and amniotic fluid are still small, and most of the early gain reflects expanded blood volume and early maternal fat stores. Many women gain less than this — and many gain nothing or lose weight due to nausea and food aversion — without adverse effect on the pregnancy.
Underweight women who do not gain in the first trimester face elevated risk of preterm birth (per the IOM review of evidence), so first-trimester gain is particularly important in this BMI category.
Second and Third Trimesters (Weeks 14-40)
For healthy-weight women, the IOM recommends a steady gain of approximately 0.8-1.0 lb/week in the second and third trimesters. The corresponding rates by BMI category are:
- Underweight: 1.0-1.3 lb/week
- Healthy weight: 0.8-1.0 lb/week
- Overweight: 0.5-0.7 lb/week
- Obese: 0.4-0.6 lb/week
The declining weekly rate across BMI categories reflects evidence that excessive gestational weight gain in higher-BMI women increases risk of gestational hypertension, preeclampsia, cesarean delivery, large-for-gestational-age infants, and postpartum weight retention. Insufficient gain in lower-BMI women increases risk of preterm birth and small-for-gestational-age infants.
Worked Example: Three Women, Same Pre-Pregnancy Height
Consider three women, all 5'5" (65 in, 165 cm), entering pregnancy at different weights and BMI categories:
Woman A: 110 lb pre-pregnancy (BMI 18.3, underweight)
- Target total gain: 28-40 lb
- Target first trimester: 2-4 lb (gain to 112-114 lb by week 13)
- Target weekly gain (weeks 14-40): 1.0-1.3 lb/week
- Estimated delivery weight: 138-150 lb
Woman B: 140 lb pre-pregnancy (BMI 23.3, healthy weight)
- Target total gain: 25-35 lb
- Target first trimester: 2-4 lb (gain to 142-144 lb by week 13)
- Target weekly gain (weeks 14-40): 0.8-1.0 lb/week
- Estimated delivery weight: 165-175 lb
Woman C: 180 lb pre-pregnancy (BMI 30.0, obese)
- Target total gain: 11-20 lb
- Target first trimester: 0-2 lb (gain to 180-182 lb by week 13)
- Target weekly gain (weeks 14-40): 0.4-0.6 lb/week
- Estimated delivery weight: 191-200 lb
Each woman is working from a different baseline BMI and therefore a different target trajectory. The clinical objective is to track gain against the appropriate category-specific range, not against a single absolute target.
Computing Your Due Date
Pregnancy dating begins from the first day of the last menstrual period (LMP), not from the date of conception. The standard clinical estimate of the due date is calculated using Naegele's Rule, named after the 19th-century German obstetrician Franz Naegele:
EDD = LMP + 280 days
The American College of Obstetricians and Gynecologists (ACOG) reaffirmed this method in Committee Opinion #700 (2017, reaffirmed 2023). The 280-day figure reflects the median duration of human pregnancy from LMP, and is approximately 38 weeks from conception (assuming ovulation at day 14 of a typical 28-day cycle).
For women with irregular menstrual cycles, ovulation-based dating (using the date of ovulation + 266 days) is more accurate. First-trimester ultrasound measurement of crown-rump length (CRL) is the most accurate dating method, with an error of approximately ±5-7 days in the first trimester.

Why the IOM Guidelines Matter
Excessive and insufficient gestational weight gain carry distinct clinical risks:
- Excessive gain is associated with macrosomia (birth weight > 4,500 g or 9 lb 15 oz), shoulder dystocia, cesarean delivery, postpartum weight retention, and increased risk of childhood obesity in the offspring. Macrosomia also raises the risk of birth injury to both mother and infant.
- Insufficient gain is associated with preterm birth (delivery before 37 weeks), low birth weight (under 2,500 g or 5 lb 8 oz), and small-for-gestational-age infants. These outcomes carry elevated risk of neonatal intensive care unit admission and developmental complications.
The IOM 2009 guidelines were calibrated to minimize the combined risk of both endpoints. They are not intended as rigid targets — actual gain varies, and a woman whose gain falls slightly outside the recommended range should not panic. Clinical evaluation by the obstetric provider, including assessment of fetal growth by ultrasound, is the appropriate response.
Common Mistakes and Misconceptions
- "Eating for two." The additional caloric need is approximately 340 kcal/day in the second trimester and 450 kcal/day in the third trimester per the Dietary Reference Intakes — roughly the equivalent of one additional snack, not a second meal.
- Using BMI cutoffs without recalculating. BMI categories apply to the pre-pregnancy BMI, not to BMI at any point during pregnancy. The BMI itself becomes uninformative as pregnancy progresses.
- Comparing to friends' gains. Two women with different pre-pregnancy BMI categories should have different gain trajectories. The appropriate comparison is to the IOM 2009 range for the woman's own BMI category, not to other women's gains.
- Restricting calories to limit gain. Caloric restriction during pregnancy, particularly in the second and third trimesters, is associated with elevated risk of preterm birth and impaired fetal growth. Weight management should focus on the quality of intake and moderate physical activity (per ACOG's guidance on exercise during pregnancy), not on caloric restriction.
- Assuming all weight gain is fat. As the breakdown above shows, the majority of gestational weight gain is water, blood, fetus, placenta, and amniotic fluid — not adipose tissue. Only about a quarter of the gain is maternal fat stores.
Conclusion
Pregnancy weight gain should be tracked against the IOM 2009 ranges for the appropriate pre-pregnancy BMI category, with the rate of gain in the second and third trimesters being the more clinically meaningful signal than the absolute total. Underweight, healthy-weight, overweight, and obese women face different recommended trajectories, and comparing across categories leads to unnecessary anxiety or false reassurance.
